Passport Information

If you do not have a passport you must begin the process to get it immediately!!. I can not stress this enough.

To obtain a password application visit your local Post Office or download it here:
http://travel.state.gov/download_applications.html

You will have to take your completed application, two recent passport photos, a picture ID, birth certificate, and a social security card, plus a check for the processing fee.

If you have your passport already, please check the expiration date and make sure your passport is valid until at least July 2004.

Once you have your passport please send me as soon as poosible:
1. Your name full as it appears on your passport
2. Date of issue
3, Place of Issue
4. Passport number
