Misc. items of importance for Mediterranean Study Tour trip members:

1. You need to be cleared with the Health Services to go on the trip.  This involves having a record of a current physical and current Tetanus and Diphtheria shot on file among other things.  Please check directly with Health Services about this requirement.  There will be a future trip meeting with staff from the Health Clinic as well. The Lee Health Clinic requires
(A) a current (within one year) physical exam on file;
(B) current (within 5 years) Tetanus and Diptheria shot required, Hepatitus A is recommended but not required;
(C) the completion of a health questionnaire form.  These are only required of STUDENTS, not faculty or non-student adults going on the trip (who sign waivers).

I can send the Health forms if you all can not attend the meeting.  Lee will actually do physical exams and shots quite cheaply for any students going on the trip.

2.  You need to sign “waiver of liability forms” before going on the trip.  These will be available from Dr. Hoffman.

3. You need to register for the summer classes you plan to take in connection with the trip (summer I).
(A)    You need to register for the trip in general by signing up for Mediterranean Study Tour using this tracking code – SSP MED
(B)    You need to select 7 hours of course credit (or no hours at all)
(C)    The choices are (a) HUM 201 section MED Foundations of Western Culture in summer II for 3 hours OR HIS 480, MED, The History and Archaeology of the Greco-Roman World in summer II for 3 hours, and ; (b) ANT/HIS 490, MED, The Islamic World in summer II for 3 hours, and (c) GST 251, MED in summer II for 1 credit hour.
You may get multiple credits in GST 251.

4. If events beyond the control of any faculty or Lee University officials cause the trip to be canceled, we will do everything possible to get as much refund as possible, but there is no express or implied guarantee that we can get any such refunds.  In other words, we can not promise ANY refund if the trip is cancelled.  If you want insurance and/or a guarantee of a refund, you should purchase “trip cancellation insurance” from a third party.  One such vendor provides information at http://www.counciltravel.com/travelinsurance/default.asp  The cost for a trip our length from this vendor is $95, and the coverage also includes limited medical and other kinds of insurance for the duration of the trip. It is strictly up to you whether you want this coverage or not, but Lee requires that you sign a waiver saying that you are willing and able to absorb the risks of not having insurance (Dr. Hoffman will have the forms) if you are not covered by your own health insurance.

5. You need to get a passport as soon as possible if you do not already have one!!

6.
